Why Does the Leaking Happen?

To understand the leak, you have to understand the plumbing. The prostate gland sits directly under the bladder, wrapping around the urethra (the tube you pee out of). Because of its size and location, the prostate acts as a natural "blocker" or secondary valve that helps keep urine in.

When the surgeon removes the prostate, that physical blockade is gone. Suddenly, your pelvic floor muscles (the external sphincter) have to do 100% of the work to keep you dry. If those muscles are weak, fatigued, or uncoordinated, gravity wins, and you leak—especially when coughing, standing up, or lifting.
